Medical Clearance Information
*Farmingdale State College will assume no responsibility whatsoever for the payment of, or authorization to pay, medical expenses resulting in injuries that occur while participating in intercollegiate athletics at Farmingdale State College.*

PHYSICAL EXAMINATIONS
If you are having a complete entrance physical examination by your own physician please have your doctor fill out the student-athlete physical form located in the paperwork above.
The Health & Wellness Center is available if you are unable to get a physical with your own physician. FOR ALL APPOINTMENTS: (934) 420-2009Â
**All paperwork including physicals, cardiac questionnaire (new/transfer athletes only), and sickle cell verification form and results (new/transfer athletes only), will be collected by the Athletic Training staff. Athletes will no longer be required to submit this to Health in Wellness in order to be cleared.**
CONCUSSION BASELINE TESTING

**The Sports Medicine department will hold baseline concussion testing at the end of each academic year and one time over the summer in conjunction with the mass physical dates.
If you are unable to make those dates you can book an individual concussion appointment with the Sports Medicine staff by emailing FSCSportsMedicine@farmingdale.edu.
Once school starts, concussion testing will be by group scheduled sessions only.
INSURANCE COVERAGE
ALL athletes are required to have medical insurance. If they do not, they can apply through the New York State Health Plan Marketplace.
